Yea, I seem to notice a new one that's only for ages 50-85. I've had the "all ages" one for awhile now. Mine (the 50-85 one) has a revision date of 11/26/2012. Maybe they lost it until now?
Funny thing is...the one for all ages says "Seniors" at the top. Go figure.

Scroll down to Downloads and lick the drop down menu to find your state. However, the state of insanity isn't there (yet).

The rate table for under 50 is included in the 0-49 application booklet.

Am I overlooking something on the 50-85 application or will they accept an insulin dependent diabetic as long as they have no complications like neuropathy?

The 50-85 one allows ol' farts to be a little fatter. This is a good thing.
