I don't know if this is the type of policy you are looking for, but...

I saw an Oxford Life single premium policy a few years ago that had a nursing home rider I liked.

I was at a Senior Expo recently. Questioning people at care facility tables, It sounded to me like costs could run 4K-6.5K per month. Impossible financial burden.

The point of that comment is, At the 100K or less level, the nursing home rider to that policy will not provide a very large portion of the policyholder's cost need. Also, the payout term is pretty short compared to the time one might need nursing home coverage.

Don't know the answers to commission and application questions.
 
Looking for WL (Not term!) that would pay living benefits including LTC and critical illnesses.
Looking for policies including coverage for $100,000 and less and easy phone/online apps.
Do you want whole life or just permanent coverage?

Because there are a ton of these built on GUL chassis.

OneAmerica and Securian have built their hybrid LTC plans on whole life chassis.

Most of the major mutuals have some kind of living benefits including LTC.

But, if you're looking for faces under 100k, Nationwide has a great non-par WL with both LTC and critical illness. They have an eApp and go down to 10k for standard and tobacco (100k for preferred/250k for preferred best).
