I know the title is a stretch! I believe Aetna/Accencdo gives an immediate decision and is moderately priced. Any others?
And, how about getting a decision without entering payment information? (I usually just enter payment as annual mode and change it after approval; people are understandbly hesitant about giving bank info over the phone before even knowing if they're approved)
