client's former employer taking forever to get this form filled out (like 60 days.....seriously). Does anyone have an alternative to this form?

having same issue. Would love to know. You'd think there be an alternative. What happens if the employer went out of business and there's no one to contact?!
 
actually had that happen with a local bbq sauce (Maul's) here ins stl a few years ago....I think the client got so mad that they just filled out the form themselves and said screw it.

I also had a federal retiree who needed the form when the gov shut down for a decent amt of time a few years ago. It took them like 6 months to get the dumb thing filled out.
 
Looking through some old posts, I found a 2017 post from an agent saying she used Creditable Coverage documents.

So, if your client knows what insurance company(ies) provided his health insurance and insurance companies will still do Creditable Coverage letters, that might be a possibility. My one effort to get a creditable coverage letter from a health insurance company in 2016 met with failure so I don't know how well it might work in this situation.
 
Status update. Constant bitching and calling the employer worked. L564 forms emailed to client today.
 
Looking at the forms I don't see where they put a Part B effective date on them. Do they just write like 10/1/21 beside where it asks if they want to sign up for Medicare Part B? Maybe I just missed it.
