OON Health Care Waiver Expired... Advice for Appeal?

Hi,

I have an employer-based health care plan.
I live more than 75 miles from a particular type of specialist, so I was granted a waiver to see a specialist, who would typically be out-of-network...

However, I didn't realize that I'd let the waiver lapse. And this, all of my claims for the past few months are being reimbursed at only the out-of-network level.

I'm trying to appeal this with my insurance company. Any advice?
