I like Ring Central. Not that it will be lower than pay-per-minute calling, but rather for it's features.
I have a "free" toll-free fax line. (it does come off of your monthly minutes).
I have what they refer to as a "Call Controller" on my desktop. I see the call coming in before it forwards to my local #. I could answer the call via my mic and speakers, but I prefer the phone.
If I receive a fax, it comes to me in pdf form already. No need to print it out unless I want to.
I think you can buy 500 minutes for around $15/month. I pay more because I need more minutes.
You can also have different extensions. For instance, if you have co-workers, they all can have a different extension.

I've used these folks for about a decade. The plan I have has 6 second increment billing. The toll-free number is $0.99 per month and either 2.7 or 3.7 cents per minute.

I also use Ring Central. The fax feature is great since I get a fair amount of faxes. Some foxes fax but I would have no way of being sure.

I have been using them for about 3 years. I use te 6 sec increments also.

Kall8 is free. I think Mark may have or still does use them. You can't beat free!
I use Opex communications.
