You increase the deductible then come in with supplemental benefits to fill in.

Or

You increase the deductible and fill in with an HRA.

Morgan White -Premium Saver plan coupled with a ppo that has copay's. Another way is to use an hsa with the employer covering the deductible. Depending on the group, most likely the employer will come out ahead. This has to be done on a case by case basis.
