I made the suggestion a while back that this forum would be a great place to have something like generic forms.
Most company sites have them. They may differ a little company by company but they are all similar. It is rare that a company makes you use theirs.
Two that I would have handy are Diabetes and Cancer then you can dial down the types of cancer.

if you just want a ball park estimate, quote him table 4-6 with banner life....But if you want to look professional, ask him all the pertinent questions so you can deliver an ACCURATE quote
