It is a violation with most State Laws for Insurance Applications unless the Carrier Files the process with the State.
IGO apps and LISA apps along with certain other apps from Apptical are used by carriers, each is filed for approval under the States where the carrier uses them.
You may use a fillable form, but it must be presented to the client as a printed document with a wet signature..... To do differently subjects you to DOI disciplinary actions and a For Cause Termination of your contracts.
Best to use what the carrier accepts and has filed correctly than take a short cut.
