I'd probably start by contacting the carrier in question. Did you "sign" the apps they have received? It could get interesting.

S I assume they are not forwarding commissions, but are gracing you with a 1099?

I would contact the carrier and get a copy of the apps. Then report them to the Carrier and the DOI. The Carrier will terminate their contract and the DOI will fine them and suspend (if not terminate) their agency license.
