Nothing needs to filled out tax wise on your end. It is an arms length legitimate transaction. Same thing as having your accountant do your taxes, or your barber do your hair.
 
Sorry for not getting back on this thread sooner. With regard to that special situation for withholding of life insurance salesperson, is that person sells primarily for only one company they are considered a statutory employee. That means that although no income tax is to be withheld, he/she is considered a common- law employee by statute for purposes of FICA, Medicare, and FUTA, and those taxes must be withheld. I got stung by that at one time working for an insurance company that issued a W-2 indicating that only the above three named taxes had been withhleld----but no income taxes. I had to cough up all the income tax without credit for anything withheld.:mad:
