The biggest change in the past decade when it comes to rural/regional hospitals and health insurance has been Obamacare, especially in the states that expanded Medicaid. Obamacare took care of most of their no-pays. Without it, many more would likely have shut down. Those hospitals are among the biggest fans of Obamacare.
