Gerber's the only other DE carrier I can think of that doesn't ask height/weight until Settlers starts taking it.

And yes they do .Example my guy would max out a 379 STD at 5'5" 295#s Unisex.

They will take a TIN only if no SSN. Phone app with no need to send in the app. I found the POS pretty easy.
