No. Some companies have their own, but I just put a couple sentences down. Have them sign and date. I also include their SSN and DOB. I also put my contact info and license number down. I have done them on a napkin or envelope. Same for policy info authorizations.

Sometimes I do not send in till after I get an inforce illustration and/or a copy of the last annual report (UL) In case I need to replace it. Or if I am going to rewrite the term or WL soon.

If the plan is a keeper, I send it in. If it is a company I do not have or want to pick up. I make sure I get the authorization.

Just make sure to inform them at the beginning of each call that Peter is deceased and you've been assigned to service their policy.

You joke but I've had a client tell me they received a call from another agent taking over their 403(b) account but didn't mention names...The client says I'm very happy with Peter...this guy says I'm sorry to inform you that he passed away recently. The client asked when did this happen and the other agent said a couple of weeks ago...

The funny part was my client responded well it didn't take too well as he called to confirm our review last night.

I finally got to the bottom that this guy had gotten to be friends with the payroll specialist in the district and this person was given a list of all the teachers contributing to a 403(b) account and this guy was calling people and telling them their agent was out of the business or dead or whatever....I imagine it would work in a lot of cases because I've run across all to many people that signed up and that was the last they ever saw their rep.

Didn't think to go to the DOI...Figured they would care less...but I did have a conversation with the business manager at the district....Didn't go as well as I would hope as the agent was a VALIC agent and was in pretty tight with the district....He wasn't in good enough to not be dumped with the rest of us when they went to a single vendor program with a vendor that had never been in the district.
 
If he put it in writing that you were deceased, it's libel. If he verbally said it, it is slander. The point is that he knew you were not deceased, but he was using the damaging misinformation to divert income from you to him.
