John, this is a new service from Google similar to other social bookmarking services out there.
I added this as soon as Google made the +1 service public, so its functionality is quite similar to Facebook Like button or Tweet this from Twitter.

You may read more about their policy here and you may also opt out of their options to collect your +1'd sites, so your contacts will not know if you +1'd a site or not.

Here is the link with information collected from +1 sites which would appear on your public profile

Just so you know, in order to +1 something, one needs to be logged in to their Gmail/Google account.

Social engagement is very important. I would recommend both using your real name and having a Google +1 button on your site. People are much more willing to trust real people rather then avatars.
