I did this lead a few years back and it didn't turn out well. They all wanted the gift card but no insurance.

a local agent of mine tried this several years ago. He knew an agent from Ohio that was involved with an agency who I believe came up with this idea. His experience was it did not net more sells than what a traditional lead card did. He did receive a higher return overall. The funny thing is that a lot of the people seemed put off that it was only $5. Goes hand in hand with the "Law of Entitlements" they are never enough or appreciated.
I have used walmart gift cards in past and so far my response rate is double that with DM. regular Dm .07 , Wal, 2.0 to 2.5%. Does anyone know a DM who has them. I'm with new IMO and they don't use it. Yeah some people just want the card. I found that to less than 30% of Face to Face.

Pretty sure I bought them through Kramer Direct a few years ago.
Any mail house can customize a lead for you though.

I know TAG sends them in their leads. The reason why I know I got a lead in the mail from TAG and It mentioned Walmart gift card.
