You may be waiting a while..... Don't they know who you are?!?
First..it takes, at least, 60 days to get an application approved. If you have any medical history whatsoever...good luck getting it issued.
Second..all applications must be done in person.
Third...my contract states I must write 50 plan C before I can write a Plan F.
Fourth...in Arkansas Plan C is $115 which is cheap.
Fifth...i dont write them!!

WTF is that about? What difference does it make what plan you and your client decide on? This company has some of the most moronic "rules" I've ever seen.
Kinda like Shuria Law of the medsupp carriers.

Not sure about their reasoning of this "rule".
I do know that for some odd reason their are alot of doctors in Arkansas that are not accepting Medicare assignment. Folks have to pay their way then get reimbursed by means of a check if they see one of those doctors. CRAZY!!
I'm sure you probably already know this but for those who don't, just because the doctor insists on payment at the time of the visit, it does not mean that the doctor does not Accept Assignment. The doctor can accept assignment and require the patient pay before leaving the office.
I "caught" a doctor in Missouri doing this. He was requiring payment before the patient left the office, filing the claim with Medicare and since he Accepted Assignment payment from both Medicare and the insurance company went directly to him.
He was collecting from Medicare, the insurance company and his patient. He was not reimbursing his patient when he received payment from Medicare and the insurance company.
The only way to know for sure if the doctor is doing this is to ask your prospect or client if they get a check from Medicare and the insurance company. If they say "no" then the doctor Accepts Assignment and is collecting from everyone.
If I have a client who's doctor is collecting before they leave the office I tell them either refuse to pay or find another doctor.
